NOA Top-15 Application Form
for Basketball

For each of the five categories below, check the point value that represents your information.  If you would like to be considered for only one of the two lists (boys or girls), then only check the point values in the list you would like to be considered for. You must forward (email or snail mail) certain documentation to this application.  Any missing documentation will result in zero points for that category.  The required documentation:  1) “Ratings Summary” and “Tournament Availibility & Assignments” page from the IHSA Officials Center  2) a copy of your schedule which lists the date, location, boys or girls, and level of contest.  When printing out your pages from the IHSA Officials Center make sure your ID# is visible.

6. Contributions to the NOA

The committee will use your answers to the following questions to determine how many points you will earn in this category.

a. How many of the sport meetings did you attend last season?

 

  b. Are you currently a board member or have been aboard member in the past?

d. Have you attended the NOA/AOS Joint meeting in the past two years?

e. Have you been a presenter at a meeting in the last three years?

Contributions to the NOA (The committee will assign your point value)

Good = 5 points
Average = 3 points
Poor = 1 point

If two or more officials have the same total point values, then ties will be broken by the higher level in the state tournament (category 2) and if needed, the higher rating (category 3).

Any official who has worked TWO state finals shall not be considered for the top 15 list, unless the amount of officials who submit applications is less than 16. If this is the case then the official may be considered for the list, but only for the lowest available spot.
